One Year Later: Psilocybin Trial Data Points to a New Kind of Depression Treatment

For most people living with treatment-resistant depression, relief has meant something taken every single day. A pill in the morning, another refill at the end of the month, and the quiet hope that this one finally works. So when a late-stage clinical trial reports that a few supervised doses of a psilocybin medicine may hold benefits for a full year, it is worth slowing down and paying attention.

On September 9, 2026, Compass Pathways released 52-week results from COMP005, one of its two Phase 3 trials of COMP360, a synthetic, proprietary formulation of psilocybin being studied for treatment-resistant depression (TRD). It is the latest step in what is shaping up to be the most important year yet for psychedelic medicine.

What Happened

COMP005 was designed in parts. In the earlier, blinded portion, participants received either COMP360 or a placebo. The new data comes from Part C, an open-label extension in which participants could receive a 25 mg dose of COMP360, knowing that is what they were getting.

According to the company’s announcement:

  • Participants originally randomized to the 25 mg arm who received an additional dose showed an average 13-point reduction from baseline on the MADRS depression scale at Week 52, with durability extending out to one year.
  • Participants who had been in the placebo arm and received their first 25 mg dose in Part C saw an average 10-point reduction, which the company says reinforces that a single dose can produce a rapid, meaningful and lasting effect.
  • Across everyone who received a 25 mg dose in Part C, 40 to 45 percent were responders and roughly 30 percent were in remission at every timepoint in the six weeks after that dose.
  • The safety profile was consistent with earlier parts of the trial, with no new safety findings reported.

Compass says its rolling New Drug Application to the FDA is underway, with the final submission expected in the fourth quarter of 2026 and a commercial launch anticipated in the first half of 2027, subject to FDA approval.

Why It Matters

Treatment-resistant depression generally describes depression that has not improved after trying at least two standard treatments. For people in that situation, options are limited and often require ongoing, frequent care. The idea behind COMP360 is different: a small number of supervised sessions, with the hope of benefits that last months rather than days.

This news also lands in a very different regulatory climate than just two years ago, when the FDA declined to approve an MDMA-assisted therapy for PTSD. Since then:

  • In July 2026, the FDA finalized its guidance “Psychedelic Drugs: Considerations for Clinical Investigations,” the first final federal guidance setting out how psychedelic drug trials should be designed. A draft had been in place since June 2023.
  • On September 14, 2026, the FDA held a public hearing on the potential future therapeutic use of psychedelic drugs, and on October 5 it requested public input on how to design early-phase clinical trials of ibogaine.

None of this approves a drug or changes the law. But taken together, it shows regulators actively building the framework that would be needed if a psychedelic medicine is approved.

The Science, in Plain English

The MADRS (Montgomery-Asberg Depression Rating Scale) is a standard clinician-rated scale used in depression research. Higher scores mean more severe symptoms, so a 13-point drop is a meaningful move in the right direction. A “responder” typically means someone whose score fell substantially, and “remission” means symptoms dropped low enough to be considered minimal.

Psilocybin is the compound found in certain mushrooms. In the body it is converted to psilocin, which acts mainly on serotonin 2A receptors in the brain. Researchers think this may temporarily loosen rigid patterns of thought, the kind of looping, self-critical patterns many people with depression know too well. In the trial setting, it is given in a controlled clinical environment with trained professionals present before, during and after the session.

A few honest caveats: these are topline results announced by the company, not yet a full peer-reviewed publication. Part C was open-label, meaning participants knew they were receiving the drug, which can influence results. And the FDA has not yet made a decision.

A Shift in Perspective

At Inner Vision Artworks, we talk a lot about perspective and the mind’s eye, the idea that how we see shapes what we live. Plato’s allegory of the cave in the Republic imagines people who have only ever seen shadows on a wall, and the difficult, disorienting turn toward the light that it takes to see things as they are.

Marcus Aurelius put it even more simply in his Meditations (Book IV, 3, in George Long’s translation): “The universe is transformation: life is opinion.”

Depression can feel like being locked facing one wall. What makes this research so compelling is not the promise of an escape, but the possibility of helping people turn around, with care, guidance and support, and see their own lives from a new angle. Science is now measuring what philosophers have long pointed toward: a change in perspective can be a change in everything.

An Important Note

This post is for informational purposes only and is not medical advice. Psilocybin, MDMA, LSD and other psychedelics remain Schedule I controlled substances under U.S. federal law and are illegal outside of approved research settings and limited state-regulated programs. COMP360 is an investigational medicine and has not been approved by the FDA. If you are struggling with depression, please talk with a licensed medical or mental health professional. If you are in crisis, please contact local emergency services or a crisis line right away.
